Basic Term Life
Creative Foam provides Basic Term Life and AD&D insurance benefits to all eligible employees through Prudential at no cost to you. The Basic Life benefit is payable to your beneficiary at the time of your death. The amount of AD&D coverage is the same as your Basic Life amount. Please see the certificate for exclusions. Contact your Human Resource representative to obtain more information on this important benefit.
Beneficiary Designation:
It is important that you confirm/update your Beneficiary Designation. Your beneficiary may be updated at any point based on your request. If you intend to name a minor as beneficiary, carriers recommend that you establish a trust.
Voluntary Life Insurance
Voluntary Life Insurance is available to non-union employees. Employees coverage by collective bargaining agreements are excluded.
Non-union employees have the option to purchase voluntary life insurance in addition to the basic life plan for yourself, your spouse, and your children. Voluntary Life insurance is provided through an insurance policy issued by Prudential. If you enroll within 31 days of your initial eligibility, coverage is guaranteed up to the Guaranteed Issue limits stated below. Those electing coverage after it was initially declined require medical underwriting and approval by the carrier before payroll deductions or coverage will begin.
Employee Optional life coverage is available in increments of $10,000, up to $150,000. Download the following rate sheet for costs.
